In addition to the benefits of massage, many people may not know that their insurance could cover the treatment if the doctor determines it as "medically necessary"; it can be a chiropractor or PCP.

Under Obamacare, section 2706 states that licensed massage therapy can be covered by medical insurance.

We suggest that people ask their doctor if massage therapy could benefit their overall treatment plan. For example, your doctor can write a 'massage prescription,' or you can download a physician referral/prescription form on my website in the "PDF Forms" section, which can be printed out for your doctor.

What steps must you take before your first medical massage therapy session?!

Step 1: Insurance Verification. We need to determine if your insurance company will pay for Massage Therapy. This is what you need:

  1. Your full name 
  2. Your date of birth
  3. Name of the insurance company
  4. Phone number of your insurance company
  5. Policy number
  6. If it is an auto injury. Would you please provide Personal Injury Protection (PIP)

Step 2: Doctor's prescription form. You can find and print the form in the PDF Forms section of this website.

Step 3: Forms for your first visit.

  1. Intake Form. For your safety and privacy, this form must be filed and found in the PDF Forms section as a link to the electrical form.
  2.  Please print out the "Confidential Health Intake Form," which can be found on my website in the "PDF Forms" section.

Step 4: Print a pain chart and fill it out on the day of your treatment. This form can be found on my website under the PDF Forms section.
